Mandatory requirements:
As per the Ministry of Health and Family Welfare (MoHFW) guidelines, Government ID ( Aadhaar card / Voter Id / Passport ) to support the current address and contact number of the suspect patient should be given at the time of sample collection.
It is mandatory for the suspect patient to possess the doctor’s prescription at the time of sample collection. The prescription should have the Doctor’s medical registration number and Doctor’s stamp.
The suspect case will also be asked about travel history and Covid-19 patient interaction (if any).

Who should be tested
Not everyone needs to be tested for COVID-19. Here is some information that might help in making decisions about seeking testing.
i. All asymptomatic individuals who have undertaken international travel in the last 14 days:
a. They should stay in the home quarantine for 14 days.
b. They should be tested only if they become symptomatic (fever, cough, difficulty in breathing)
c. All family members living with a confirmed case should be home quarantined.
ii. All symptomatic contacts of laboratory-confirmed cases.
iii. All symptomatic health care workers.
iv. All hospitalized patients with Severe Acute Respiratory Illness (fever AND cough and/or shortness of breath).
v. Asymptomatic direct and high-risk contacts of a confirmed case should be tested once between day 5 and day 14 of coming in his/her contact.
vi. Direct and high-risk contact include those who live in the same household with a confirmed case and healthcare workers who examined a confirmed case without adequate protection as per WHO.
